NECC Liability Waiver

Everyone who comes into our facility needs to have a liability waiver on file. If your child is planning on coming into the facility on their own for the first time, please fill out the waiver below ahead of time. Children over the age of 10 may use the facility on their own so long as they have a completed waiver, but only those age 13 and older can attend fitness classes and use exercise equipment.
